Overall Objectives of Position
The Case Manager–Behavioral Health is a core member of a collaborative team approach. The primary goal is to ensure that an individual reaches their optimum level of wellness and functionality within their environment. They are responsible for supporting and coordinating the mental and physical health care of patients. This job description is not an all-inclusive list of duties and responsibilities of this position. The Case Manager–Behavioral Health is expected to perform all duties and responsibilities necessary to meet the goals and objectives of applicable programs.
The Case Manager–Behavioral Health is expected to live the Purpose and Values of the Poarch Band of Creek Indians and will go beyond the call of duty.
Primary Responsibilities of the Case Manager- Behavioral Health
- Administers comprehensive, evidence-based screenings and assessments to identify behavioral health needs and other risk factors that may require further intervention.
- Provides case management services to patients with mental health and substance abuse disorders.
- Collaborates with family members in understanding and making them part of the case/treatment plan.
- Responsible for assisting supervisor/therapist with inpatient admissions and requirements.
- Responsible for assisting supervisor with daily operations such as day to day calls, group facilitation, etc.
- Utilizes strength-based and trauma-informed approach with patients to support their treatment and recovery.
- Monitors patient access and participation in treatment and assist with managing any barriers to treatment.
- Ensures completion of all documentation in Electronic Health Records (EHR) according to procedure.
- Participates in Quality Improvement activities including data collection, tracking and analysis.
- Participates in all assigned meetings, staff development, and trainings as required.
- Provides training/education/outreach to address the stigma of mental health issues.
- Performs other duties as assigned by the appropriate person.

Education/License/Certification and Experience Requirements
- Must have one of the following:
- a. Bachelor’s Degree in Psychology, Sociology, or Social Work field, or
- b. Associate’s Degree with two (2) years in a clinical setting.
- Must possess certification of “Mandatory Reporter Training” or obtain certification during the first week of employment.
- Experience with Electronic Health Records preferred.
- Employment experience in a Behavioral Health/Day Treatment care setting or addictions certified preferred.
Skills Required
- Absolute confidentiality, adhering to HIPAA guidelines and the Privacy Act.
- Demonstrates flexibility in job duties and schedule in order to accommodate situations that may occur.
- Practical knowledge of assessment tools and working knowledge of other drug and alcohol treatment agencies, programs, and resources.
- Must have working knowledge of computers with the ability to learn various software programs including but not limited to Electronic Health Record.
- Demonstrated proficiency in English, spelling, punctuation and writing composition. Must successfully pass applicable knowledge skills and abilities exams.
- Demonstrate excellent customer service and be able to collaborate with physicians, patients, staff, and community, able to communicate verbally in a tactful, effective manner.
- Ability to demonstrate sensitivity to cultural diversity and workplace harmony.
- Must be able to maintain a good working relationship with people in a cross-cultural setting.
Additional Requirements
- Ability to work odd and irregular hours, as needed; flexible in regard to accommodating emergency visits as needed.
- Must possess a valid state driver’s license and insurable driving record according to Tribal insurance guidelines.
- Must successfully pass the required criminal and character background check.
- Ability to travel and participate in required training, leadership development, and other events.
- Ability to adequately and successfully perform all duties and responsibilities of this position.
